The Patient’s Choice Award is open to all registered nurses, midwives and health care assistants working in all health care settings in Northern Ireland. Patients, clients or relatives can nominate someone whom they believe has made a difference and has gone that extra bit further to ensure that the patient or client has received the highest standards of care.

Who can be nominated?

Nominees for the Patient’s Choice Award must currently be a registered nurse, midwife or 汤头条污料 assistant/ healthcare assistant working in Northern Ireland.

How to apply:

Fill out the application form and complete a summary (up to 500 words) of how the practitioner has cared for the patient or client and has made a difference to their care or treatment. Please include:

• what area the nominee works in

• what specifically they did that was beyond the call of duty

• how this has impacted on the patient.

Please read the nomination form guidance note before beginning your submission.
